Rehabilitation Designed to Help the patient Return Home Stronger

Prairieview’s skilled team of therapists is here for you!

Inpatient Rehab

  • Shortly after admission, our therapists will assess mobility, strength, balance, ability to perform daily activities, communication, and swallowing if needed. They also review medical history and current limitations.

  • Based on the evaluation, the therapy team creates individualized goals (e.g., walking a certain distance, dressing skills, improving swallowing). This plan is coordinated with nursing staff and physicians.

  • Residents start regular therapy sessions, 5 days a week for short-term rehab patients.

  • Progress is tracked, and therapists adjust exercises and goals based on improvement or setbacks. The care team will hold periodic meetings to review progress.

  • Therapy is integrated into daily routines. Nursing staff reinforce therapy goals.

  • As therapy progresses, the team plans the next steps. This could mean discharge to home, continuation of therapy at a lower intensity, or long-term care support.

  • Overall, the goal of therapy is to maximize independence, safety, and quality of life as quickly and effectively as possible.
